Purchase Agreement

Seller (Maria Reed) and Buyer ____________________________ agree to enter this contract under the following agreement:

Seller and Buyer agree that the above described puppy is a purebred Labrador Retriever registered with AKC.
The above puppy is being sold with_____________ registration,_________ breeding rights.
AKC registration papers will be delivered to buyer on day of sale.

Purchase Price: Buyer agrees to the total payment of
$________________ for the purchase of the above described puppy. A *deposit of
$________________ has been placed on described puppy, with a remaining balance of
$________________ to be paid in full on the day of pick up/delivery.

*Deposits are non-refundable. Order of deposits received determines order of buyer choice.

SHIPPING: If a puppy should be shipped, BUYER agrees to pay shipping cost, cost of crate, and mandatory health certificate.

HEALTH GUARANTEE:

SELLER guarantees the puppy to be free of any communicable disease and in good health for a period of 5 days after the puppy goes home. During that time, the BUYER agrees to have the puppy checked by a board-certified veterinarian within 5 days from pick up:____________ and maintain all required vaccinations as set forth in the health records. BUYER agrees that any other pets living in the same place as the puppy will be up to date on vaccinations to validate the agreement. BUYER agrees that the puppy will be housed in a safe, clean, and dry environment with shelter from extreme elements/temperature with with access to clean water and quality puppy kibble, given affection, adequate exercise, and medical care upon injury or illness. Failure to meet these terms invalidates health guarantee. BUYER agrees to notify breeder within 5 business days and provide an original signed copy of statement of health from a board-certified veterinarian. Minor treatable problems such as diarrhea, coccidiosis, parasites, etc are common problems in puppies, especially under the stress of entering a new home and will not be covered by this warranty, although every effort is made to prevent this.

SELLER *guarantees that if the puppy is found to have genetic hip dysplasia during the first year from birth, seller will replace puppy. The BUYER agrees to provide x-rays and report from Orthopedic Foundation for Animals (OFA) that the puppy is dysplastic and return the puppy to the seller for agreement to be valid.

SELLER *guarantees is the puppy is found to have genetic PRA of the eye that would cause blindness during the first year, SELLER would replace puppy. Report from a Board Certified Veterinary Ophthalmologist is required, and puppy must be returned to validate this agreement.

*Guarantee is to replace the puppy when one comes available, upon the return of the original puppy.

Purchase agreement: The puppy is warrantied for the purpose of pet/companion, even though it may be suitable for other purposes. BUYER releases breeder and agrees to hold harmless for any liability, injuries, or damages to persons or property by this puppy. BUYER assumes responsibility for health, temperament, and appearance of puppy upon purchase. It is the BUYER’S responsibility to maintain vaccinations and de-worming, heartworm and flea/tick prevention to validate health warranties. Puppies are de-wormed regularly at and receive their first rounds of vaccinations with vet check at 6 weeks. SELLER will provide health certificate from the vet before leaving for home.

This agreement may not be altered, changed, or amended except in writing, signed by both parties.
